Appointment Scheduling Policy

Ultimate Image Cosmetic Medical Center is always “buzzing” with activity, so appointments are required. We encourage you to schedule appointments well in advance, especially prior to major holidays, to ensure we can provide the desired services. A Visa, Mastercard, Discover, or American Express credit card will be required at the time of scheduling to secure your appointment(s). We observe strict privacy policies and will not disclose this information to any other party. Your credit card number will be securely kept in your history file. A $50 deposit is required for scheduling an appointment, and will be deducted from the balance of services rendered. We kindly ask that you review our cancellation policy before finalizing your appointment. By scheduling, you acknowledge and accept the terms of this policy. As a courtesy, we offer one complimentary 15-minute consultation to new clients. Please note that existing clients who request a repeat consultation about a service already discussed but decide not to schedule a follow-up appointment to receive the service, will be charged a consultation fee of $50.

Appointment Reminder

As a courtesy to our clients, we will call and/or send you a message reminder prior to your scheduled service. If you choose not to provide us with a current cell phone number or email, we will be unable to send you a reminder. Should the appointment reminder system fail for any reason, and you do not receive an appointment reminder, it is still your responsibility to manage your appointment and adhere to the cancellation policy.

  

Arrival

Please arrive 10-15 minutes prior to your scheduled appointment to allow ample time for relaxation and completing necessary paperwork that may be required. Late arrivals may limit the time of your treatment, as appointments end at a scheduled time to avoid delaying subsequent client appointments.

  

Grace Period

 

We understand that delays happen. We offer a grace period of 15 minutes beyond the scheduled appointment time. If you arrive more than 15 minutes late for your appointment, we may need to shorten or reschedule your treatment. We encourage you to notify us as soon as possible if you anticipate being late, so we can make every effort to accommodate your late arrival.

Rescheduling Due to Late Arrival

 

Should you arrive late for a scheduled appointment and time does not allow us to perform your treatment, your appointment can be rescheduled, however, the $50 deposit fee will be forfeited.

Cancellation, No-Show, and Rescheduling Policy

At Ultimate Image Cosmetic Medical Center, we understand that sometimes schedule adjustments are necessary. However, when appointments are cancelled or rescheduled without adequate notice, we miss the opportunity to fill openings with clients on our waiting list to receive services. If we do not receive the required notice for adjustments and cancellations the following fees will be applied to your card.

24-Hour Notice Required

 

We request that you provide a minimum 24-hour notice if you need to cancel or reschedule your appointment. Special surgical appointments scheduled for Mondays must be communicated by 3pm on Friday to avoid cancellation fees. To cancel or reschedule your appointment, please call/text us at 727-799-7000 or send an email to info@ultimateimagetampabay.com. Voicemails, text messages, and emails are acceptable forms of cancellation if received at least 24 hours prior to your appointment time. Notification given at least 24 hours prior to your appointment will receive no charges.

Late Cancellations

 

Cancellations made within 24 hours will result in the $50 deposit fee being forfeited.

No-Show Fee

 

If you fail to attend your scheduled appointment without prior notification, the $50 deposit fee will be forfeited and an additional $50 no-show fee will be applied to the credit card on file.

​

Multiple No-Shows

 

Repeated no-shows (3 no-shows) may result in a requirement to prepay services for future appointments.

Return Policy

 

Products

 

We believe in the exceptional quality of our products and are confident you will enjoy them as much as we do. However, if you are not completely satisfied with your purchase from Ultimate Image Cosmetic Medical Center, you may return the unused and unopened item with a receipt dated within 14 days for a credit or exchange. Please note that refunds are not available. We do not accept any items returned after 14 days. Due to health regulations, we cannot accept returns of opened or used products.

Services

 

All treatments and packages are non-transferable and non-refundable and expire one year from the purchase date. Any unused services in your package will not be refunded but can be used as a future credit.

Prepaid Packages & Promotions

 

Prepaid packages and promotional services are non-refundable. Please see the Cancellation, No-Show, and Rescheduling Policy. Any applicable fees will be deducted from your pre-pay balance.

Gift Certificates

 

Gift certificates are not redeemable for cash and cannot be replaced if lost or stolen.

Referral Policy

 

Referrals are greatly appreciated and are the highest compliment you could give us! To show our appreciation, if the patient you refer purchases a treatment, you will receive a $25 off your next neuromodulator treatment (Botox®, Dysport®, Xeomin®, Daxxify®) or $50 off your next dermal fillers. It’s our special way of saying thank you for spreading the love!

  

Final Disclosure

Although we make every effort to keep our website and spa menu updated, please note that prices and services are subject to change at any time. We love having you as a customer, but we do reserve the right to refuse service at any time, to anyone, for any reason. At Ultimate Image Cosmetic Medical Center, we appreciate your understanding and cooperation in helping us maintain the quality of service that you and all our clients deserve.
